Reader's Advisory - Book Annotation Form

This is an easy-to-use form to keep track of details and elements of appeal in a particular book. Make copies (paper or electronic) for staff to use, keeping track of what they read and find interesting about a particular book. This form can be collected as a quick reference or posted online as part of a blog or wiki. Make changes to fit your library - but please note: consistency is important!
